What exactly is meant by sagging breasts?

Over time, primarily due to gravity, most women experience a sagging of breast tissue, as well as of the surrounding skin and nipple height. The latter is largely responsible for the overall shape. Essentially, several basic shapes can be distinguished:

Normal-sized breasts, nipples not significantly sagged

A relatively minor procedure—removing skin circumferentially around the nipple, or only in the upper area (crescent shape)—can significantly improve the overall shape. This can be done under local anesthesia.

Normal-sized but sagging breasts, nipples significantly lowered.

By tightening the glandular tissue and possibly removing excess skin, especially around the nipple area, the shape is improved. This procedure is somewhat invasive and requires general anesthesia.

Breasts that are too small and sagging; nipples not or only slightly lowered.

Tissue needs to be added here. This is usually done in the form of additional implants (link augmentation), or by autologous fat injection (provided there is enough donor tissue) or by injection with hyaluronic acid (Macrolane).

Breasts that are too large and sagging at the same time

The correction is performed as part of a reduction surgery, either using scar-sparing techniques or conventionally (anchor-shaped scar).

Before the operation

For approximately seven days before a breast lift, you should avoid taking aspirin, sleeping pills, and consuming alcohol. Both aspirin and other painkillers delay blood clotting and can lead to unwanted bleeding.

Smokers should drastically reduce their nicotine consumption four weeks before the procedure to avoid jeopardizing wound healing.

Procedure of the operation

The breast lift is performed under general anesthesia. If you wish, you can remain in the recovery room for a few hours after a recovery period and then be taken home. However, an overnight stay in the clinic is also possible.

The incision technique depends on the desired result. Often, removing a single ring-shaped strip of skin (peri-miliary lift) is sufficient. The scars lie around the areola and are therefore almost invisible. If this is not enough, incisions can be made from the areola downwards or in the breast crease (lift using a scar-minimizing technique).

If the breasts are already very small before the planned lift, breast implants can be used to achieve the desired result.

For very large breasts, which often begin to sag at a young age, a breast lift alone is usually insufficient, as the weight of the breasts would soon cause them to sag again. To achieve optimal results, a breast lift for large breasts should be combined with a breast reduction (reduction mammoplasty).

In all breast surgeries, care is taken not to damage the sensitive nerves and blood vessels of the nipple, in order to preserve both sensitivity and the ability to breastfeed.

The wound is then closed with self-dissolving sutures (i.e., suture removal is not necessary). A tight bandage supports the breast after the operation.

After the operation

It takes several months for the nipples to regain normal sensitivity. Follow-up appointments at the doctor’s office will be scheduled after approximately two weeks. A full result is usually achieved after about six months.
